Additional Requirements and Information
- Qualified Disabled Veteran's and Qualified Disabled Active Duty Military ONLY: Military physician's affidavit stating 50% of greater service-connected disability or is certified by the U.S. Department of Veteran's Affairs as having incurred a 50% or greater service-connected disability.
