Liability Relinquishment

All participants must agree to this release. I hereby consent to receive medical treatment which may be deemed advisable in the event of injury or illness during this activity. I acknowledge there are risks of physical injury or illness during this activity. I acknowledge there are risks of physical injury to participants and I agree to assume the full risk of any injuries, damages or loss, regardless of severity, which I may sustain as a result of participating in activities connected or associated with this program. I waive and relinquish all claims that I, my insurer or my family may have against Alaska Department of Fish and Game and its officers, agents, servants and employees from claims from injuries, damages, or loss which I may have or which accrue to me on account of my participation in the above program.

If I am registering a participant under the age of 18, I certify that I am the parent or legal guardian of the minor participant(s), or have legal authority to consent on their behalf. I acknowledge the risks associated with participation and agree to assume full responsibility for the minor participant(s). I consent to their participation in this activity and agree to waive and relinquish any and all claims on their behalf against the Alaska Department of Fish and Game and their officers, agents, servants, and employees for any injuries, damages, or loss arising from participation in this program.

Photo Release

By typing "Yes" in the field above, I give the Alaska Department of Fish and Game permission to use my or the above-named participant's (if participant is under 18 years of age) photograph for informational or educational purposes without incurring any debts or liabilities of any kind.
